It is the responsibility of the applicant to provide the Registrar with all necessary information and documentation needed to assess the applicant’s eligibility for registration.
1. Proof of current provincial dietetic association/regulatory body membership. Once the applicant completes the CDM application form, the College will contact their outgoing regulator directly asking for details of the applicant’s membership status, by completing a labour mobility verification form.
2. Provide current quality assurance/continued competence documentation on their activity for the past year, or since they submitted last to their outgoing regulator.
