It is the responsibility of the applicant to provide the Registrar with all necessary information and documentation needed to assess the applicant’s eligibility for registration.
1. Proof of current provincial dietetic association/regulatory body membership. This should be in the form of a letter/verification form to be sent to CDM directly from the regulator or association providing details of the applicant’s membership status.
2. Provide current quality assurance/continued competence documentation on their activity for the past year, or since they submitted last to their outgoing regulator.
